Requirements:
Access and set up your internet video visit by calling 203-877-3221
New clients must have a doctor-patient-client visit before utilizing Telehealth
Current clients have access to Dr. DeForge’s Telehealth diagnostic opinions; testing recommendations; and therapeutic advice

All has been simplified to schedule your Video Visit with Dr. DeForge:

• After requesting a video appointment, one of the SSV nurses will provide all the information that you will need to proceed.
• Payment is completed [$55.00] at the time of scheduling.
• You must be sure you have a computer or Smartphone with a camera; speakers; and microphone
• Take time and write down all of the questions that you want reviewed with Dr. DeForge a week before your visit
• Please send all lab work and photography if available to DoctorDeForger@yahoo.com one week before your visit; this will be time saving and allow a seamless consultation with Dr. DeForge
• If you have had a visit at another veterinary hospital within the last 6 months prior to your video visit with Dr. DeForge please upload your previous doctor’s Medical Report to DoctorDeForge@yahoo.com
• You can also upload any pertinent documents to our APP Silver Sands Veterinary. This is a free APP available at the APP Store.
