Drop-Off Appointment Checklist

Are you having trouble making time to visit Dr. DeForge?

We are pleased to offer our clients a drop-off concierge service. The drop-off concierge service for the day is provided as a no-charge daycare visit.

Drop your pet off between 8:00 and 9:00 AM. We’ll take care of your pet like he or she was our own for the day! When you arrive, one of our SSV nurses will greet you. Complete your online Drop-Off Appointment Checklist before dropping off your pet. New clients: Fill out the New Patient Form in addition to the Drop-Off Checklist.

An SSV veterinary nurse or a doctor will call you after we have completed a thorough physical examination and assessment of your pet. We will discuss our findings and recommendations over the phone. If your pet is ill, we will discuss how additional testing (blood work, X-rays, or other testing) may help us determine the problem. We will also discuss with you our recommendations regarding medications and other ways you can help to keep your pet healthy.

Dr. DeForge and his associate veterinarians will never perform any testing or treatments without giving you an estimate and getting your informed consent. Therefore, it is essential that we be able to reach you by phone during the day. Make sure your cell phone is on and calls are not blocked. We will ask that you sign an Informed Refusal if you do not want a recommended test or service to be performed.

You can arrange a time to pick up your pet before 5:00 PM Monday through Friday and Saturday before 1:00 PM. We will email a medical report with a summary of our findings and recommendations for you!

Schedule a Drop-Off Appointment

Because of COVID-19, clients are not allowed into our hospital at this time.

We ask that you call us when you arrive, and we will tell you when to bring your pet to our Secured and Safe Locked Foyer for Admission. The same procedure occurs with pick-ups. Please call us when you arrive in our parking lot at your designated pick-up time, and we will let you know when your pet is available to be taken home from our Secured and Safe Locked Foyer for Discharge.

An approximate pick-up time will be given on admission. Only after the doctor’s exam and permission is given for the recommended care, i.e., testing and treatment can an estimate and precise pick-up time be assigned.
